This CPN post will carry a caseload of clients referred to the Slough Pathways Outreach Team, and will liaise closely with senior clinicians in the development and implementation of care packages/ risk assessment and management for users with severe and enduring mental illnesses.

 

The post holder will be required to provide care management to service users and people significant to the clients and will work in a way that values people’s rights and enables them to make informed choices.

 

The client group has diverse and complex needs, having difficulty engaging with mainstream mental health services. We work with a team approach to enable a variety of skills and interventions to be available to both clinicians and service users
